Information for Health Professionals referring to ADH Primary Health (Outpatient) Services

 

Alexandra District Health offers a range of nursing and allied health outpatient services to Murrindindi Shire residents. These services include;

 

Physiotherapy (excluding TAC and Workcover)

Occupational Therapy

Dietetics

Speech Pathology

Diabetes Education

District Nursing

Cardiac Rehabilitation

Pulmonary Rehabilitation

Social Work

Counselling/Psychology

 

How to refer?

Referrals can be emailed to ADHIntake@adh.org.au or sent via eFax to 03 57510334

 

When referring please provide the following information;
Patient Information summary – DOB, Address, Phone/mobile, Medicare, GP, Primary Contact/Next of Kin
Reason for Referral
Discharge Summary and/or Medical History

If you have any queries for waitlist times please contact Intake Officer on 03 5772 0908

 

Post Acute Care services:  Please contact Seymour Health 5735 8065 to commence referral process.

 

Aged Care Services (Commonwealth Home Support Program):

For people over 65 (and Aboriginal and Torres Strait Islanders over the aged of 50) ADH offers District Nursing, Physiotherapy, Dietetics and Occupational Therapy services via the Commonwealth Home Support Programme (CHSP). This allows people to access services at home or at our centre.   Referral to these services is only accepted via the My Aged Care system. Health professionals can refer patients for aged care services via the website Make a Referral | My Aged Care  or phone 1800 836 799.
