At Alexandra District Health our Primary Health team provide a range of services and programs that can be tailored to your needs.

Our primary health services are provided at our Alexandra site, however some services are provided at our campus in Marysville or as home visits.

The Counselling and social services team supports social, emotional, and mental health.  They assist you with care planning, supportive counselling, brief psychotherapy, targeted psychological strategies, advocacy, and referrals.

Our diabetes educator will assess your needs, understand what matters most to you, and create a personalised diabetes management plan that will help you manage your diabetes day-to-day, prevent complications and manage complications if they happen.

Our diabetes educators also offer education for the general community to increase awareness of diabetes.

ADH’s team of Physiotherapists and Allied Health Assistants are there to help you recover after a life-changing event such as a bad fall or support you to manage a chronic condition. How to access this service – click on this link How to access our Primary Health services

Gentle Exercises

Is suitable for those who have limited weight bearing ability, experience chronic pain or prefer chair-based exercises. This gentle approach to building strength is tailored to each individual, with some participants improving and progressing into the Stall the Fall program.

Stall the Fall

The balance and falls prevention program is for people with a history of falls, have a fear of falling or have been identified by their GP as high-risk of falling which is limiting their participation in daily activities.  This program is also for people who just feel they have lost some strength and confidence in this area and are seeking some support.

GLA:D

A 6-week program for people living with hip and/or knee osteoarthritis, that aims to reduce pain, amount of pain relief required and the progression of symptoms that may lead to surgery.

Lungs in Action

A maintenance exercise group for those with long term breathing problems. This program is delivered by a Lung Foundation trained worker and is recommended after the completion of the Pulmonary Rehabilitation, although this is not a prerequisite.

Our dietitian works closely with you to offer advice that suits your personal health and lifestyle goals.  They will give you diet advice for conditions such as diabetes, heart health, gastrointestinal issues, weight management and poor appetite due to cancer.

Our dietitian also offers education for the general community to increase awareness of healthy eating and eating well for specific health conditions like diabetes and heart disease.

Our occupational therapist (OT) works with adults and uses different techniques and equipment to help you keep, regain or improve your independence. They can see you at our health service or at home.

Physiotherapy services help you recover from injury/ illness, reduce pain and stiffness, increase mobility and prevent further injury. Our physiotherapists help you manage existing health conditions you may have and will design a treatment plan specific to you. They also assist people to use aids and appliances to help with mobility, rehabilitation and independence.
